What does a mastopexy operation involve?

Mastopexy is a surgical procedure designed to lift sagging breasts, eliminate sagging, and reshape them.

This procedure is suitable for correcting breast ptosis, caused by aging, pregnancy, significant weight changes or the presence of previous implants that have descended over time.

During mastopexy, the breast is repositioned in the center of the chest and its appearance is reshaped.

In addition to lifting the breast, this surgery rounds its shape and corrects possible alterations in the areola and nipple.

A significant advantage of mastopexy is that patients can resume their daily activities as little as 24 hours after the operation.

Dr. Sordo does not use drains or bandages, and arm mobility is complete from the first moment.

Advantages of a breast lift surgery

If you decide to undergo a mastopexy at Dr. Sordo's clinic, you will be choosing an advanced technique that offers multiple benefits compared to more traditional methods:

⇒ No need for drains or bandages: Due to the small incision and gentle tissue handling, bruising is rare. The use of microdissection needles during surgery reduces bleeding, resulting in less swelling and a more comfortable recovery. Drains are not necessary with Dr. Sordo's technique; avoid them to reduce the risk of infection.

⇒ Laser scar treatment: After surgery, I apply a laser session to the scar to help it heal better and become as inconspicuous as possible. The goal of the laser is to minimize the scar's appearance; it doesn't make it disappear completely.

⇒ Private room admission and personalized follow-up: Patients enjoy an overnight stay in a private room for greater comfort and security, with administration of intravenous antibiotics. Dr. Sordo conducts a personalized follow-up the next day to check on your progress.

⇒ Rapid return to social life: Most patients can resume their social activities between 24 and 48 hours after the mastopexy, thanks to the rapid recovery and full mobility of the arms from the first day.

Types of mastopexy in Madrid

Ask for an appointment

There are different types or techniques to lift sagging breasts in our patients.

Thus, we can distinguish:

Periareolar mastopexy

Periareolar mastopexy is a technique in which an incision is made around the areola to lift and reshape the breasts . This technique is suitable for cases where the breasts have only slight sagging and breast ptosis is mild to moderate.

One advantage of this technique is that the scars are usually less visible. However, if you need to correct a very sagging breast, this technique may not be the most suitable.

Periareolar mastopexy

Vertical Mastopexy

Vertical mastopexy, also known as a " lollipop lift ," is a technique in which a vertical incision is made down to the breast crease and another around the areola . It is ideal for correcting moderate ptosis and providing a more youthful and lifted breast shape.

One advantage it has over other techniques is that it offers more natural results and less scarring than the inverted "T" technique. However, it may not be sufficient to correct very sagging breasts with severe ptosis.

Mastopexy (breast lift) in Madrid

Inverted "T" Shaped Mastopexy

This technique involves incisions around the areola, a vertical incision down to the breast crease, and another horizontal incision in the same crease . It is suitable for cases where the breasts are very sagging, as it provides a significant lift and reshaping.

The advantage of this technique is that it corrects very sagging breasts, but it can leave more visible scars and prolong recovery.

Augmentation mastopexy with implants

In some cases, a mastopexy can be combined with breast implants to increase breast volume and projection. This is recommended when breast volume is very small or there is excess skin.

This technique is suitable for women who want to lift sagging breasts and increase their breast size at the same time. The advantage of this technique is that it offers fuller, more youthful results, but it may require a longer recovery time.

We recommend you read this blog post about the different types of implants.

Mommy makeover (ideal for after childbirth)

Some women experience significant changes in their figure during pregnancy and after childbirth wish to regain their body shape.

It is a cosmetic procedure that combines several surgeries to restore a woman's body after pregnancy and childbirth.

It usually includes a combination of surgeries such as abdominoplasty to tone the abdomen and achieve a flat and firm stomach, mastopexy to lift and firm sagging breasts, and liposuction to remove excess accumulated fat.

However, it is important to keep in mind that a mommy makeover is a complex procedure that requires significant recovery time and proper post-operative care.

Price of a mastopexy or breast lift in Madrid

Also known as a standard breast lift , simple mastopexy focuses solely on lifting the breasts without adding extra volume through implants.

It is performed to correct sagging breasts and improve their position and shape without changing their size.

In all cases, the repositioning of the areola and nipple is considered according to each patient's anatomy. And if there is excess skin, we take care of that as well.

During the consultation, Dr. Sordo will analyze your body structure and advise you according to your individual characteristics.

Our goal is for you to feel beautiful and confident in your breasts, minimizing any impact on your personal and professional life, with a result that still reflects who you are. Areola sensitivity may change for a few months; during your consultation, I'll explain what to expect in your specific case.

The price of a mastopexy can be affected by several determining factors.

First consultation and 3D simulation

During your first consultation at our clinic in Madrid, you will be greeted directly by the doctor, not by a commercial intermediary.

During this consultation, the doctor will examine your anatomy and listen to your wishes and expectations regarding the breast lift you have in mind.

As part of this consultation, we will offer you an on-site 3D simulation , which is included in the consultation price. This simulation will provide you with an honest and realistic representation of how your breasts will look after cosmetic surgery.

It is an important tool to give you confidence in your decision. In addition, you can view it later comfortably at home, on your mobile or computer, in a completely private manner.

Our goal is to achieve a natural result, but the most important thing for us is to listen to you, understand what you are looking for, and tell you frankly what part of that is achievable.

Preoperative mastopexy

The pre-operative assessment for a mastopexy serves to detect in time anything that increases the risk of the procedure.

Once you make the decision to undergo surgery, we will coordinate the date for the operation with you.

Before the appointed day, we will perform a series of preoperative tests.

These tests are essential to ensure that you are in optimal condition for the procedure and are included in the agreed budget.

We will provide you with a referral to our partner laboratory, where you can perform the necessary tests, and the results will be sent directly to us, you do not need to travel to collect them.

Preoperative tests include:

  • Analysis to evaluate coagulation.
  • An electrocardiogram to check heart health.
  • A breast ultrasound to have a detailed analysis of the current state.

Once the results are ready, our anesthesiologist will review them thoroughly to ensure everything is in order. Additionally, we will meet with you again for a consultation before the surgery.

At this appointment, we will take photographs, go over the details of the procedure, and provide you with all necessary documentation.

The mastopexy operation

Preparations at the Hospital

On the day of the operation, you must be admitted to the Moncloa Hospital in the morning.

Anesthesia

We use general anesthesia, specifically the "TIVA" (Total Intravenous Anesthesia) technique , which offers a more comfortable and safe experience for our patients.

During the procedure, you will feel as if you are in a restful sleep , which facilitates both the procedure and the subsequent recovery.

And what about anesthesia? Here's what kind of anesthesia is used in each surgery : local, sedation, or general, and why TIVA, without anesthetic gases, is used when general anesthesia is needed.

Moncloa Hospital

All our procedures are performed at the HLA University Hospital Moncloa.

For those seeking maximum discretion, our clinic offers an exclusive VIP service. This includes direct access to your private room from the garage and additional support for your companions during your hospital stay.

During the Operation

A mastopexy typically lasts between 2 and 3 hours . During this time, our medical team, led by Dr. Sordo, will lift and reshape your breasts to improve their shape and size.

The "24h Recovery Laser" technique by Dr. Sordo will be used, which minimizes surgical trauma and reduces recovery time.

Throughout the entire procedure, our anesthesiologist, Dr. Bourgeois, will continuously monitor you throughout the intervention.

Admission Night

After the operation, we will transfer you to the recovery room for 1 or 2 hours , before returning you to your private room for the night.

Spending the night in the hospital allows for monitoring during the first few hours, which are the most risky for bleeding.

Additionally, a companion may stay with you overnight to provide additional support.

This process is designed to ensure your comfort and safety at every stage of the mastopexy operation.

Postoperative mastopexy

Thanks to our " 24h Recovery Laser " technique, you leave the operating room wearing only a bra, without the need for drains or bandages.

This allows you to resume your social life in just 24-48 hours. You will feel tired, so we recommend a short period of sick leave, from 4 to 7 days depending on your activity and how you feel.

The day after surgery, Dr. Sordo will visit you personally to assess your condition, make an initial evaluation of the result and check that you can move your arms normally before discharging you from the hospital.

At home, it is important that you follow these instructions:

  • Sleep on your back.
  • Continue treatment for healing.
  • Take pain relievers and antibiotics as prescribed.

With our "24h Recovery Laser" technique, you won't need any additional aftercare. From the very beginning, you'll notice no bruising and spectacular results.

It's normal to feel discomfort similar to chest pressure during the first few days, but we'll provide you with pain relievers so you can continue with your daily activities.

You may experience a temporary feeling of numbness if the areola and nipple have been repositioned, but this will disappear over time.

The initial incision may be red and swollen, but it will clear up during the first year.

Our laser in the operating room helps reduce the size and improve the appearance of the scar by 30%.

In addition, the stitches are absorbable, so they do not need to be removed and the healing process does not cause discomfort.

One week after the operation, you can start sleeping on your side if you don't feel pain. You can move normally, use your arms and carry weight using your legs.

For more information about breast surgery scars, we recommend reading our blog post ( Breast Surgery Scars ) , where you will find additional details about the recovery process and post-operative care.

Revisions after a mastopexy

Our commitment to your well-being doesn't end with the surgery. During the first year, we'll provide ongoing monitoring to ensure everything progresses as expected . After this initial period, we recommend an annual ultrasound and scheduled regular checkups to assess your progress.

At 30 days after mastopexy

It's normal for some swelling to persist, but it will gradually decrease. Thanks to the laser treatment, the scarring will be less noticeable, and the stitches will have completely dissolved . At this stage, you can resume wearing any type of bra and begin gentle, progressive cardio —brisk walking, cycling, swimming, running, gradually increasing the intensity—along with isometric exercises that don't involve the pectoral muscles.

6 months after mastopexy

The last restriction is lifted: the powerful isometric chest exercises —bench presses, dips, and weighted flyes—which are the only exercises you have to wait six months for. If you opted for a mastopexy with implants, you'll notice that the texture of your breasts becomes more natural and that the swelling has decreased considerably; when the implant is placed in the subfascial plane , chest exercises can be resumed after just one month.

Upon reaching the year of surgery

We will perform a complete review that includes a high-resolution ultrasound to ensure the perfect integration of the implant in the case of mastopexy with implants.

In addition, we will take some photographs so you can appreciate the change from the beginning of the treatment to the present moment.

After this final check-up, you will be discharged and encouraged to continue with annual ultrasounds to maintain ongoing monitoring of your breast health.

Risks and limitations of mastopexy

A mastopexy is a surgical procedure and, like all surgeries, carries risks. We prefer that you be aware of these risks before making a decision, and that we discuss them during your consultation.

What can happen

In addition to the risks of any surgery performed under anesthesia (bleeding, infection, scarring problems), mastopexy can result in slight asymmetries, changes in nipple and breast skin sensitivity, and scars that can take up to a year to fully heal. When an implant is added, the inherent risks of breast implants are also present, including capsular contracture and the possibility of a second surgery in the future. Scheduled checkups are essential for detecting and treating these issues early. We explain how scars heal in this scar guide . Should any unforeseen problems arise, we will support you every step of the way until they are resolved.

What we cannot promise you

A mastopexy lifts and reshapes the breasts, but it doesn't fix the result permanently: the skin and tissue continue to change over time, with pregnancies and weight fluctuations. The scar improves, but it doesn't disappear, and the final shape is only visible once the swelling subsides, not before.

Frequently asked questions about mastopexy

Will the result be natural?

Yes. In a mastopexy our aim is to correct sagging breasts, improving the size and shape of the breast. That is why it is essential to seek proportion with the rest of your physiognomy. To do this, we carry out a prior study and a three-dimensional simulation. If you need breast implants, we will test different types of prostheses until we find the one that best fits your anatomy.

Can I lead a normal life after a mastopexy?

Yes. With the “24h Laser Recovery” technique, you will leave the operating room directly with a bra, without the need for drains or bandages, and you will be able to return to your basic daily activities within 24 hours. Although you will feel fine and lead a normal life, we recommend taking 4-7 days off work to ensure recovery and not subject the body to unnecessary stress.

Do you use general anesthesia? Is it safe?

Yes. We use “TIVA” (Total Intravenous Anesthesia), a new technique that does not use anesthetic gases and will not cause headaches, vomiting, or nausea.

It is the safest anaesthetic technique currently available, with which you will be fully monitored and controlled. In addition, we have had our own anaesthetist on our team for over 10 years.

Are there different types of breast prostheses?

Yes. Prostheses can be round or anatomical. Depending on your preferences and your physiognomy, we will use one or the other. We will decide with you which one you prefer during the consultations before the intervention.

Does breast lift surgery require hospitalization?

Yes. We do this for greater security because this way we can monitor you in the hours that follow. The next morning Dr. Sordo will visit you to check on your progress and, if everything is okay, he will discharge you from the hospital.

Will I be able to breastfeed after a mastopexy?

Yes. Breast lift surgery does not affect breastfeeding, the quality or quantity of milk produced by the breast. The prosthesis is placed behind the pectoral muscle and does not come into contact with the mammary gland, so it will not affect breastfeeding.

In the case of repositioning the areola and nipple, breastfeeding is not affected either, since the tissues are treated with the utmost care and are not disconnected from the ducts.

Will I be able to have mammograms after breast lift surgery?

Yes. It is important that you inform the radiologist that you have breast implants, if applicable, so that the appropriate procedures can be performed.

Which implants are right for me?

These are the factors we consider when choosing one implant or another:

  • If the patient is not thin and has a significant amount of skin and fat in the upper part of the breast, there will be no difference between the two prostheses. It will make no difference whether a round implant or an anatomical one, since they will look the same from the side.
  • If the patient is thin, it will be noticeable from the side which type of implant she has chosen, since the round prosthesis will give a more marked transition at the beginning of the prosthesis, at the top.
  • If the patient has an underlying abnormality in the shape of her breast (tuberous breasts, for example), it is recommended that she select an anatomical implant, with the aim of normalizing the shape of her breast as much as possible and giving it a more natural appearance.
  • If the patient is looking for a natural result, it is advisable to opt for an anatomical implant, since the appearance will be more similar to that of a non-operated breast.

Of course, we take into account your personal preferences regarding size (without exceeding the maximum recommended) and a more natural or more pronounced neckline.

What is “TIVA” anesthesia?

“TIVA” (Total Intravenous Anesthesia) is a high-quality anesthesia. Its innovative technique avoids the use of anesthetic gases and the well-known headache, vomiting, nausea and discomfort that anesthetic gases can cause.

This is a type of anesthesia that requires more dedication from the professional than common anesthesia with gases, but it greatly increases the well-being and safety of the patient. In our clinic we have our own anesthesiologist and we take the utmost care of these types of details.

The patient is monitored throughout the intervention with the so-called BIS (Bi Spectral Index) that measures sleep constantly.

Even when it comes to general anesthesia, our patients' experience with TIVA anesthesia has been excellent thanks to Dr. Bourgeois, a highly experienced professional who has mastered the technique perfectly.

Should breast implants be replaced every 10 years?

No, no implant needs to be replaced unless it is ruptured, which can easily be checked with an annual ultrasound.

The average lifespan of the highest-end implants (the ones we use) is around 20 years. There is no need to replace what has not broken. Remember to have your ultrasound every year and keep in touch with us for your check-up.

Where are breast implants inserted?

In most cases, the approach to inserting breast implants will be via the inframammary fold (under the breast crease), with the incision size being 2,5 to 3 cm.

It can also be done on the areola. In any case, we will treat your scar with laser and reduce it by 30% so that no one will notice it.

What does the scar look like after a mastopexy?

At Dr. Sordo's clinic, we treat scars to keep them minimal, regardless of the approach point. Once the mastopexy is finished and while you are still under general anesthesia, we will apply our scar laser.

It's a 1210 nm surgical laser applied to the already closed incision. In a randomized, double-blind trial of breast reduction surgery, where one breast was treated and the other served as a control, the treated scar was flatter and less rough after one year. It doesn't make the scar disappear; the goal is to make it as inconspicuous as possible.

Where are breast implants placed?

With few exceptions, the implant will be placed under the pectoral muscle.

This is the safest and most comfortable option for the implant and the patient.

When can I play sports after a mastopexy?

For the first month, no exercise is done, but that doesn't mean being sedentary: after 24 hours, you can raise your arms overhead and bring your shoulder blades together for 30 seconds, once an hour, and walk for 30 minutes a day. From day 7 onwards, the 1 kg weight limit is lifted and movement is unrestricted: you can move your arms whenever you want, without repeating the exercise every hour.

After a month, you can resume gentle, progressive cardio—running, swimming, cycling—and isometric exercises that don't involve the chest muscles. Only heavy isometric chest exercises are suspended for six months.

How much does a mastopexy cost in Madrid?

From €8.600 to €10.400 depending on the procedure: with implants, with polyurethane implants, or with capsulectomy. The final price is determined during the consultation, once I assess the amount of excess skin and whether implants are necessary.

With or without an implant?

It depends on what you're lacking. If there's excess skin and volume, it can be lifted without an implant. If, in addition, the upper part of the breast is empty, the implant is what restores that volume; a lift alone won't create it.

What are the scars like in the first year?

Mastopexy leaves a scar around the areola and, depending on the case, a vertical incision and another in the inframammary fold. For the first couple of months, the scars are red and hard, then gradually fade over the first year. I use the superomedial pedicle for its vascular safety, and laser treatment on the scar after the surgery.

Are drains installed?

I don't usually leave a drain in a mastopexy. When the case requires it—a capsulectomy, for example—it's evaluated, and I'll tell you before the surgery.
