Comprehensive postpartum surgery with breast lipofilling, liposuction and mastopexy: before and after

Pathology: 35-year-old woman after pregnancies with desires to recover breast aesthetics WITHOUT using implants, and to improve body contour in the area of the abdomen, flanks, saddlebags and inner thighs.
Planning: We decided to perform an inverted-T mastopexy to improve the breasts and reduce the size of both areolas, restoring symmetry. In addition, we performed tumescent liposuction of the flanks, inner thighs, saddlebags, and abdomen. Thanks to this technique, we were able to use a large portion of the extracted fat for breast lipofilling in the "social" area: the décolletage and medial poles.
Result: We achieved a natural result with tumescent liposuction used on the flanks, inner thighs, saddlebags, and abdomen of the patient. Thanks to breast lipofilling, we were able to increase breast volume.
Why the superomedial pedicle
All my inverted-T reductions and mastopexies involve placing the areola on a superomedial pedicle. This is the portion of tissue that keeps it alive as it is lifted, and this pedicle maintains its blood supply via two routes: the internal mammary artery and the intercostal perforating arteries. The largest published series show a favorable complication profile with this approach, even in large reductions. In periareolar mastopexy, a pedicle is not necessary because the areola is barely displaced.
