Which implants are right for me?
These are the factors we consider when choosing one implant or another:

What is “TIVA” anesthesia?
“TIVA” (Total Intravenous Anesthesia) is a high-quality anesthesia. Its innovative technique avoids the use of anesthetic gases and the well-known headache, vomiting, nausea and discomfort that anesthetic gases can cause.
This is a type of anesthesia that requires more dedication from the professional than common anesthesia with gases, but it greatly increases the well-being and safety of the patient. In our clinic we have our own anesthesiologist and we take the utmost care of these types of details.
The patient is monitored throughout the intervention with the so-called BIS (Bi Spectral Index) that measures sleep constantly.

Will I have to change the implants again?
No, no implant needs to be replaced unless it is ruptured, which can easily be checked with an annual ultrasound.
The average lifespan of the highest-end implants (the ones we use) is around 20 years.
There is no need to replace what is not broken. Remember to have your ultrasound every year and stay in touch with us for your check-ups.
Where are breast implants inserted?
In most cases, the approach to inserting breast implants will be via the inframammary fold (under the breast crease), with the incision size being 2,5 to 3 cm.
It can also be done on the areola. In any case, we will treat your scar with laser and reduce it by 30% so that no one will notice it.
What does the scar look like after a second breast operation?

Once your breast augmentation surgery is complete, and while you're still under general anesthesia, we'll use our laser to treat scars. It's a 1210 nm operating room laser applied to the closed incision. In a randomized, double-blind trial of breast reduction surgery, where one breast was treated and the other served as a control, the treated scar was flatter and less rough after one year. It doesn't make the scar disappear completely; the goal is to make it as inconspicuous as possible.
Where are breast implants placed?
With few exceptions, the implant will be placed under the pectoral muscle.
This is the safest and most comfortable option for the implant and the patient.

For the first month, no exercise is done, but that doesn't mean being sedentary: after 24 hours, you can raise your arms overhead and bring your shoulder blades together for 30 seconds, once an hour, and walk for 30 minutes a day. There is one exception: if the secondary surgery has required capsulectomy, micropolyurethane prosthesis and drains , arm mobility is not forced until the drains are removed.
After one month, gentle and progressive cardio—running, swimming, cycling—and isometric exercises that don't involve the pectoral muscles are resumed. Only heavy isometric pectoral exercises are postponed for six months ; once the implant is in the subfascial plane, they can begin after one month.

Popcorn capsulorrhaphy: closing the pocket without sutures
When an implant has shifted within its pocket—outward, downward, or toward the center—the problem is almost never the implant itself. It's the space that contains it, and that space needs to be closed where it's excess.
The classic methods involve capsular sutures, capsular flaps, or mesh. The technique I use is called popcorn surgery, developed by the Swedish surgeon Charles Randquist: instead of suturing, the capsule is coagulated in small, pointed sections using a protected forceps and an electrocautery knife. The capsule retracts upon itself—hence the name—and the pocket is closed where needed.
The practical advantage is significant: the pocket is adjusted without sutures, without traditional capsulorrhaphy, and without raising capsular flaps . This greatly simplifies revision surgery, which is why it is constantly used in implant replacements, displacement corrections, and explants.
Regarding its performance, there is a published series of 149 patients and 266 breasts. The indication was implant malposition in 61,3% of the breasts. Complications occurred in 14,7%, and 6% required some surgical revision. The authors conclude that the technique provides pocket control and stability with low complication and revision rates ( Calobrace MB et al. Aesthetic Surgery Journal. 2020;40(1):63-74 ).
It's not a straightforward technique, because no revision surgery is. It's a cost-control tool with reasonable costs for what's required.
