Periareolar mastopexy. What is it?
Mastopexy is a surgical procedure that corrects the appearance and even the volume of the breasts . Although this procedure is often associated only with breast implants, the reality is quite different. The female breast suffers the effects of time, weight gain and loss , and, above all, breastfeeding . In almost all these cases, the nipple tends to droop , sitting at or below the inframammary fold . This necessitates a specific procedure called periareolar mastopexy.

To return the nipple to its original position, Dr. Gustavo Sordo uses three different mastopexy techniques that are directly related to the type of nipple prolapse. These are:
- Around the areola. This type of intervention is performed when the breast sagging is mild. It consists of a circular incision that surrounds the areola of the breast, removing the skin that remains between it and the incision. It is very useful when a woman wants to reduce the size of the areola to give her breasts a more youthful appearance.
- Under the areola. It is performed when the breast sags moderately. To carry it out, Dr. Gustavo Sordo works around the areola of the breast, making a vertical scar a couple of centimetres above the breast crease. In this way, the sagging of the skin and the mammary gland is corrected to lift it and give it the desired volume.
- At the level of the breast groove. It is necessary to resort to this when the breast sags severely. In this case, the doctor makes an incision around the areola, as well as a vertical and a horizontal incision in the crease of the breast. This reshapes the breast, giving it an unbeatable appearance.
In all cases, periareolar mastopexy can be combined with breast augmentation . This would not require two separate procedures, as the visit to the clinic could be combined into a single surgery.

Periareolar mastopexy has a tremendous advantage—the scar is hidden within the areola—and a well-known risk: if it is tightened too much, the scar widens and the areola becomes deformed. That's why we are strict about its indications and reserve it for mild to moderate ptosis, where the nipple-areola complex has only risen a few centimeters. When the sagging is more significant, forcing a periareolar incision yields poor results: in those cases, a vertical incision or an inverted T incision is appropriate, and we explain this during consultations, even though the scar may be more daunting at first.
When sagging is accompanied by a lack of volume, we combine this procedure with implants. And in patients who prefer not to wear implants, we create an autologous prosthesis using the Ribeiro technique , remodeling their own glandular tissue and securing it with a resorbable Vicryl mesh in the prepectoral plane ; to prevent the upper pole from appearing empty or flattened, we add a small amount of lipofilling . It's a very rewarding surgery, provided the indication is correct.
