Rhinoplasty on S-shaped dorsum and mixed skin: before and after
Case fact sheet
- Patient: Female, 42 years old
- Reason: aesthetic: I wanted a smooth, straight back
- Starting nose: S-shaped hump on mixed skin, with the radix at a good height
- Technique: ultrasonic rhinoplasty structural: hump reduction with ultrasound, spacer grafts (spreader grafts) and septal extension graft
- Septum and turbinates: septoplasty and turbinates in the same surgery
- Approach: Reduce the hump and leave a straight back, without any inward curve
- Result: straight and smooth back, without the hump and without a slide effect
- Entry: 1 night
- Anesthesia: general type TIVA
- Duration: 3 hours long.
A 42-year-old woman with an S-shaped hump on combination skin. She wanted a smooth, straight bridge, without the inward curve that creates a sloping profile. I performed an ultrasonic structural rhinoplasty with spacer grafts and septal extension: the bridge is now straight and the nose is feminine and natural.

Reason for consultation
He asked for a smooth, straight back. He said it clearly: no inward curve, no slide effect, no concave profile, no sunken mid-back.
What I observed when evaluating it
An S-shaped hump and mixed skin. With one important detail: its radix wasn't low. An S-shaped back usually starts from a low radix, which is why it needs to be closely monitored when reducing the hump. This wasn't the case here, and that allowed for some leeway.
What was important to preserve
The height of the radix. Reducing an S-shaped hump without taking care of the starting point of the back is what ends up in a sunken profile, exactly what she didn't want.
My surgical approach
Ultrasonic structural rhinoplasty with spreader grafts and septal extension graft. This is the same approach as in other cases of S-shaped dorsal dermis published on this website: when the problem is the same, the solution is the same. Age does not change the technique.
Why I chose this technique
Reducing the hump opens up the roof of the back. Spacer grafts reconstruct it and keep it straight and at its proper width; the septal extension graft supports the tip and fixes its position, so that the profile does not depend on how it heals.
What I did during surgery
I reduced the hump using ultrasonic instrumentation, reconstructed the dorsum with spacer grafts, and placed a septal extension graft at the tip. In the same surgery, I performed the septoplasty and turbinate reduction.
What is observed in the result
A straight and smooth back, without the hump and without inward curvature, with a feminine and natural result.
