Ethnic rhinoplasty on a very wide nose and thick skin: before and after
Case fact sheet
- Patient: Woman
- Reason: aesthetic
- Starting nose: A very wide ethnic nose, with thick skin and a small hump, quite deceptive
- Technique: ethnic rhinoplasty ultrasonic structural: wing reduction (allotomy), spacer grafts (spreader grafts), septal extension graft and SMAS tip resection to define
- Septum and turbinates: septoplasty and turbinates in the same surgery
- Approach: narrow the base, give the tip the definition that the thick skin hid, and achieve a Barbie-like back.
- Entry: 1 night
- Anesthesia: general type TIVA
- Duration: 3 hours long.
A very good case of an ethnic nose: very wide, with thick skin and a small, rather deceptive hump. I reduced the alae with an allotomy, constructed the dorsum with spacer grafts and the tip over a septal extension graft, and thinned the tip skin with a SMASectomy to show definition. We were aiming for a Barbie-type dorsum.

Reason for consultation
I wanted a narrower, more defined nose, with a Barbie-like bridge.
What I observed when evaluating it
A very wide ethnic nose, with thick skin, and a small but rather deceptive hump: in profile it looked more than it was.
What was important to preserve
Harmony with its features. In an ethnic nose, it's not about changing its origin, but about narrowing and defining it without the nose clashing with the face.
My surgical approach
Structural ultrasonic rhinoplasty: allotomy to narrow the base, spacer grafts on the dorsum, septal extension graft at the tip, and tip SMASectomy to define it under the thick skin.
Why I chose this technique
With such a wide base, the allotomy is what truly narrows the nose when viewed from the front. The thick skin conceals the cartilage's structure; therefore, the tip requires the support of a septal extension graft and, sometimes, thinning of the skin with a tip SMASectomy, a procedure that must be performed carefully because it can compromise the tip's blood supply.
What I did during surgery
Using ultrasonic instrumentation, I worked on the hump and reconstructed the dorsum with spacer grafts; at the tip, I performed a septal extension graft and SMASectomy; and I reduced the alae with an allotomy. In the same surgery, I performed the septoplasty and turbinate reconstruction.
What is observed in the result
A narrower nose, with a defined tip despite the thick skin and a soft, Barbie-like bridge.
