Rhinoplasty on a very wide nose with a high dorsum and radix: before and after
Case fact sheet
- Patient: Female, 28 years old
- Reason: aesthetic
- Starting nose: very wide nose, with a raised bridge, a high radix, and tension
- Technique: ultrasonic rhinoplasty structural: reduction of the dorsum and radix with ultrasound, assembly of the dorsum by components (dorsal split), spacer grafts (spreader grafts) and septal extension graft
- Septum and turbinates: septoplasty and turbinates in the same surgery
- Approach: narrow the nose, lower the bridge and radix, and slim the aesthetic lines of the bridge
- Entry: 1 night
- Anesthesia: general type TIVA
- Duration: 3 hours long.
A 28-year-old woman presented with a very wide nose, a high dorsum, a high radix, and tension. Using ultrasound, I reduced the dorsum and radix and constructed the dorsum using a split technique, which allows for the refinement of the dorsal lines. Spacer grafts and a septal extension graft completed the procedure, resulting in a nose that was very well reduced with this technique.

Comment by Dr. Gustavo Sordo
Reason for consultation
He inquired about the size of his nose, which was very wide and had a high bridge.
What I observed when evaluating it
A very wide nose with a raised, tense bridge and a high radix.
What was important to preserve
The proportions of your face. A wide, high nose cannot be corrected simply by lowering the bridge: if the lines of the bridge are not narrowed at the same time, it will appear lower but just as wide.
My surgical approach
Reduce the dorsum and radix with ultrasound, assemble the dorsum by components ( dorsal split ) and provide support with spacer grafts and a septal extension graft.
Why I chose this technique
A very wide nose with a high bridge is effectively reduced using spacer grafts and septal extension. Component-based bridge reconstruction allows for the refinement of the dorsal lines, and ultrasound enables precise reduction in both the bridge and the radix.
What I did during surgery
I reduced the dorsum and radix using ultrasound, assembled the dorsum in components with spacer grafts, and placed a septal extension graft at the tip. In the same surgery, I performed the septoplasty and turbinate reduction.
What is observed in the result
A narrower nose, with a lower bridge and radix, thinner dorsal lines, and without the tension of before.
