Ultrasonic rhinoplasty with hump and wide tip: before and after

Case fact sheet

  • Patient: 20 year old woman
  • Starting nose: hump on the back and wide tip
  • Technique: Structural rhinoplasty with ultrasonic technique, along with septoplasty and turbinate surgery
  • Anesthesia and duration: total intravenous (TIVA), about 3 hours
  • Approach: Lower the hump and refine the tip, aiming for a straight profile and a natural result
  • Result: 4 months after the operation

A 20-year-old woman with a prominent hump on the bridge of her nose, visible in profile, and a wide tip, most noticeable from the front and from below. She underwent structural rhinoplasty in Madrid using an ultrasonic technique : the bone of the bridge was reshaped with ultrasound to precisely reduce the hump, the tip was narrowed and supported, and the septum and turbinates were corrected during the same surgery.

Photos taken 4 months after the operation

In profile, the bridge becomes straight and the tip is more defined; from the front and in the basal view, the tip appears narrower. These are the same before and after views, with the same lighting and framing, including the smiling profiles. You can see more results of straight and natural-looking noses , other noses with humps that have been operated on , and more rhinoplasty cases in women.

What I found

The patient's nose was deviated to the left, on an asymmetrical face: if you look at the eyes, the left one is higher than the right. This is seen constantly in consultations and should be mentioned before surgery, because no one is perfectly symmetrical. Skulls aren't, and the nose rests on that base. In the basal view, there is also a dislocated caudal septum, significantly deviated and asymmetrical.

What did I do?

An ultrasonic structural rhinoplasty with asymmetric spreader grafts and septal extension graft to reposition the caudal septum.

Why this technique

If the nose is asymmetrical, the grafts cannot be symmetrical: an identical spreader on each side maintains the deviation instead of correcting it. Compensation is necessary. The case could also have been resolved with preservation.

What to look for in the photos

From the front, the axis of the nose. And, to the side, the facial asymmetry remains, because the face is not operated on. In the basal view, how the base is repositioned when correcting the caudal septum.

If you're looking at this case for a specific reason.

Not all noses need ultrasound, nor do they all benefit equally. In our ultrasonic rhinoplasty guide, we explain in which cases the outcome varies.

Do you want to know if this can be done in your case?

The first consultation is with Dr. Sordo, either at the Chamberí clinic or via video consultation. You'll leave knowing if you're a candidate, which technique is right for you, and how much it costs.
