Two pregnancies and successive weight changes had caused the patient's breasts to sag quite noticeably, with asymmetry, with one breast being larger and more sagging than the other. The areolas, for their part, were excessively large.

The patient wanted to symmetrize and correct the sagging of her breasts, to recover the appearance she had before her pregnancies. She did not want an implant.

Intervention: We perform an “inverted T” mastopexy, in which we manage to reposition the areolas, reduce their size, symmetricalize the size of both breasts, achieve fine and camouflaged scarring thanks to a tension-free closure and obtain the result desired by the patient. To achieve greater projection, we use the Lyacir Ribeiro technique, known as “Ribeiro type II”, in which the breast tissue is worked so that it rises and generates a “breast autoimplant” effect that leaves the breast more projected and full.

Why the superomedial pedicle

All my inverted-T reductions and mastopexies involve placing the areola on a superomedial pedicle. This is the portion of tissue that keeps it alive as it is lifted, and this pedicle maintains its blood supply via two routes: the internal mammary artery and the intercostal perforating arteries. The largest published series show a favorable complication profile with this approach, even in large reductions. In periareolar mastopexy, a pedicle is not necessary because the areola is barely displaced.

Regarding this intervention

When there is both volume loss and sagging, an implant alone is not enough: the areola must be lifted and skin removed. That is a mastopexy.
