Reduction rhinoplasty for large hump and wide nose: before and after
Case fact sheet
- Patient: Female, 39 years old
- Reason: a prominent hump and a very wide nose when viewed from the front
- Starting nose: V-shaped back with a large hump, fairly wide nose, bulbous tip, and thick skin
- Technique: structural rhinoplasty with ultrasonic instrumentation: hump removed and dorsum narrowed with ultrasound, spacer grafts (spreader grafts) to control width without losing the nasal valve and septal extension graft to support and define the tip
- Septum and turbinates: Septoplasty and turbinate reduction in the same surgery, as part of structural rhinoplasty
- Approach: to reduce the overall size of the nose while maintaining harmony, feminine angles, correct nasal proportions, and respiratory function
- Result: two and a half years after the operation
- Entry: 1 night
- Anesthesia: general type TIVA
- Duration: 3 hours long.
A nose with a V-shaped bridge, a large hump, thick skin, and a rather wide forehead, which stood out in the overall appearance of the face. Ultrasonic structural rhinoplasty with spacer grafts and septal extension graft, with photos taken two and a half years later.
Photos taken two and a half years after the operation
The "after" photos are from her last check-up in January 2021, two years and seven months after the surgery. As with all the other cases, we show the long-term result, which is what really matters to those considering surgery. From the front, the difference in width is clearly visible; from the side, the bridge is straight, without the hump; and the tip follows the reduction of the rest of the nose.

Comment by Dr. Gustavo Sordo
Reason for consultation
He had a prominent hump and a very wide nose when viewed from the front.
What I observed when evaluating it
A nose with a V-shaped bridge and a large, rather wide hump, with a bulbous tip and thick skin. The combination made the nose stand out considerably from the rest of the face, both in profile and from the front.
What was important to preserve
Harmony. The goal was to reduce the overall size of the nose while maintaining appropriate feminine angles, correct nasal proportions, and respiratory function.
My surgical approach
A structural rhinoplasty with ultrasonic instrumentation: spacer grafts to control width after removing the hump, and a septal extension graft to support the tip.
Why I chose this technique
Ultrasonic rhinoplasty allows for the controlled removal of a large hump and narrowing of the bridge. Spreader grafts prevent the nasal valve from losing function when the width is reduced. And the septal extension graft provides firm support to the tip, allowing it to proportionally match the reduction in the rest of the nose.
What I did during surgery
I removed the hump and reduced the width of the dorsum using ultrasonic instrumentation, reconstructed the dorsum with spacer grafts, and placed a septal extension graft to support the tip. Septum and turbinates were also addressed in the same surgery. General anesthesia (TIVA) was used; the procedure took approximately three hours, and the patient was admitted overnight.
What is observed in the result
Two and a half years later, the difference in width is clearly visible in the frontal photo. In profile, the bridge is straight, and the nose is smaller overall, with feminine angles while maintaining its breathing function.

Regarding this intervention
Not all noses need ultrasound, nor do they all benefit equally. In our ultrasonic rhinoplasty guide, we explain in which cases the outcome varies.
