Ultrasonic rhinoplasty with deviated septum and mentoplasty: before and after one year
Case fact sheet
- Patient: Female, 23 years old
- Reason: aesthetic and respiratory
- Starting nose: Deviated nose, with a large bridge, and a caudal dislocation of the septum that covered the left nasal cavity
- Technique: structural rhinoplasty with ultrasonic instrumentation: bridge lowered and dorsum reconstructed with spacer grafts (spreader grafts), septal extension graft and asymmetric maneuvers to center the axis
- Septum and turbinates: septoplasty —correction of caudal luxation— and turbinates in the same surgery, as part of structural rhinoplasty
- Chin: Mentoplasty with anatomical implant Implantech, size M, in the same surgical procedure
- Approach: Center the axis and free the left nostril without losing naturalness, and balance the profile with the chin
- Result: one year after the operation
- Entry: 1 night
- Anesthesia: general type TIVA
A deviated nose with a prominent bridge and a dislocated septum that blocked her left nostril. Structural rhinoplasty with ultrasonic instrumentation and mentoplasty with implant in the same surgery: the axis corrected, the left nostril cleared, and a profile in which the nose and chin are finally balanced.
Photos one year after the operation
The "after" photos are from thirteen months, the last check-up. There was another one at six months, but the later one is shown because the tip and base of the chin continue to refine throughout the first year, and the chin finishes settling. This is the same reason why it's wise to be wary of results from just a few weeks. Same framing, same lighting, and same before-and-after views.

Comment by Dr. Gustavo Sordo
Reason for consultation
He had trouble breathing through his left nostril and didn't like his nose: he was looking for a solution that was both functional and aesthetic.
What I observed when evaluating it
A deviated nose with a prominent bridge. The inferior view clearly showed a caudal dislocation of the septum: the septal edge was displaced towards the left nostril, obstructing it and causing the patient's breathing difficulties. It also contributed to the nasal axis being crooked.
What was important to preserve
The naturalness and the rest of the proportions of her nose: correcting the axis and opening the airway without the nose looking operated on.
My surgical approach
A complete profiloplasty: structural ultrasonic rhinoplasty, with correction of the caudal dislocation of the septum, and mentoplasty in the same surgery to balance the profile.
Why I chose this technique
To correct the dislocation and center the axis, spreader grafts and a septal extension graft were placed using asymmetrical techniques, as the deviation was not symmetrical. Ultrasonic instruments allowed for precise reduction of the nasal bridge and reconstruction of the dorsum over the spreader grafts. Mentoplasty added what rhinoplasty alone could not provide: balance between the nose and chin in profile.
What I did during surgery
A structural ultrasonic rhinoplasty with septoplasty and turbinate reduction: I corrected the caudal septal deviated septum, reduced the dorsal hump, placed the spacer grafts and the septal extension graft, and performed the necessary asymmetrical maneuvers to center the nasal axis. In the same procedure, I performed a mentoplasty with an Implant, size M. The procedure was performed under TIVA general anesthesia and required an overnight hospital stay.
What is observed in the result
After one year, the nose has a centered axis and a straight bridge, and the left nostril is clear, with confirmed functional improvement. The chin, with more projection, balances the profile.

Her experience one year after rhinoplasty and chin augmentation
In her one-year check-up, the patient recounts in this video how she experienced her rhinoplasty and chin implant: the complex she had before the surgery and how she looks now.
