Rhinoplasty with S-shaped dorsum and tip without projection: before, simulation and after
Case fact sheet
- Patient: Female, 27 years old
- Reason: aesthetic: I wanted a thinner tip
- Starting nose: S-shaped back and tip lacking projection and definition
- Technique: ultrasonic rhinoplasty structural with spacer grafts (spreader grafts) and septal extension graft; tapered tip hanging the domes of the septal extension graft
- Septum and turbinates: septoplasty and turbinates in the same surgery
- Approach: straighten the back and give the tip the projection and definition it lacked, making it thinner
- Simulation: 2D simulation before surgery, compared to the final result
- Entry: 1 night
- Anesthesia: general type TIVA
- Duration: 2 hours and a half
A 27-year-old woman presented with an S-shaped dorsal humerus and a tip lacking projection and definition. She requested a thinner tip. I performed an ultrasonic structural rhinoplasty with spacer grafts and septal extension, and refined the tip by hanging the graft domes. The final result is shown compared to her simulation.

Before, simulation and after
Before the surgery, we created a 2D simulation of her nose. Here you can see, from the front and in profile, the initial nose, the simulation, and the final result.

Reason for consultation
He asked us for a thinner tip.
What I observed when evaluating it
An S-shaped back and a point lacking projection and definition.
What was important to preserve
A feminine and natural nose. Refining the tip couldn't be done at the expense of its support: a tip without projection that also thins without support ends up drooping.
My surgical approach
Standard structural ultrasonic rhinoplasty: spacer grafts on the dorsum and a septal extension graft at the tip. To achieve the thinner tip she requested, I suspended the domes of the septal extension graft.
Why I chose this technique
The septal extension graft gives the tip the projection it lacked, and hanging the domes from it allows for refining and fixing its position simultaneously. The spacer grafts maintain the dorsum straight and with its width after correcting the S-shape.
What I did during surgery
Using ultrasonic instrumentation, I corrected the S-shaped dorsum and placed spacer grafts. At the tip, I placed a septal extension graft with the domes attached to it. In the same surgery, I performed the septoplasty and turbinate reduction.
What is observed in the result
A straight back and a thinner, projected and defined tip, very similar to what we had simulated before operating.

Regarding this intervention
Each nose is planned from scratch: what worked for this person may not work for you. At Rhinoplasty in Madrid , we explain how the technique is chosen and what the price includes.
