Rhinoplasty in a large S-shaped hump with a drooping tip and thin skin: before and after
Case fact sheet
- Patient: Female, 38 years old
- Reason: aesthetic: the easel, which had bothered him for years, and the drooping toe
- Starting nose: large S-shaped hump, drooping tip pointing downwards, tension between the columella and the lip, and thin skin
- Technique: ultrasonic rhinoplasty structural with hump reduction, spacer grafts (spreader grafts) and septal extension graft
- Septum and turbinates: septoplasty and turbinates in the same surgery
- Approach: Remove the hump, leaving a smooth back under a thin skin, raise the tip and release the tension of the columella over the lip
- Entry: 1 night
- Anesthesia: general type TIVA
- Duration: 2 hours and a half
A 38-year-old woman presented with a large, S-shaped dorsal hump, a drooping tip, and thin skin. Using ultrasonic rhinoplasty, I reduced the hump and reconstructed the dorsum with spacer grafts, and raised the tip over a septal extension graft, which also relieved the tension under the columella, above the lip.

Reason for consultation
He wanted to get rid of the easel, which had bothered him for years. The tip also needed to be corrected; it was drooping and pointing downwards.
What I observed when evaluating it
A large S-shaped hump, a drooping tip, and thin skin. And tension at the junction of the columella and the lip.
What was important to preserve
A smooth back. Under thin skin, any irregularity is noticeable, so the removal of such a large hump had to be very precise and the back had to be well reconstructed.
My surgical approach
Structural ultrasonic rhinoplasty: reduce the hump and reconstruct the dorsum with spacer grafts, and raise the tip with a septal extension graft.
Why I chose this technique
Ultrasound allows for the precise reduction of a large hump, leaving the bone smooth—an important feature under thin skin. Spacer grafts maintain the dorsum's straightness and width, while the septal extension graft holds the tip in its new position and relieves tension on the lip.
What I did during surgery
I reduced the hump using ultrasound, placed spacer grafts, and a septal extension graft to lift and support the tip. In the same surgery, I performed the septoplasty and turbinate reduction.
What is observed in the result
The profile without the hump, the upturned tip, and an improvement in the tension under the columella, at its junction with the lip.
