Rhinoplasty on a wide nose with a hump: before and after at six years old
Case fact sheet
- Patient: Female, 28 years old
- Reason: Aesthetic: remove the hump and refine the nose
- Starting nose: Very wide Latin nose, with open base and nostrils; hump on the back and long nose, with a wide and poorly defined tip
- Technique: Structural rhinoplasty with ultrasonic instrumentation: hump reduction with ultrasound, spacer grafts (spreader grafts) on the dorsum and septal extension graft at the tip, and allotomy to narrow the base of the nose
- Septum and turbinates: Septoplasty and turbinate reduction in the same surgery, as part of structural rhinoplasty
- Approach: narrowing a very wide base with an allotomy, removing the hump and refining the tip by giving it support, without the nose ceasing to be its own
- Result: six years after the operation
- Entry: 1 night
- Anesthesia: general type TIVA
- Duration: 3 hours long.
A very wide, Latin-style nose with flared nostrils, a hump on the bridge, and a broad tip. Ultrasonic structural rhinoplasty with alartomy to narrow the base. The "after" photos are from more than six years after the surgery.
Photos taken six years after the operation
The "after" photos are from her June 2025 follow-up, more than six years after the surgery, the last session available. The change is most noticeable from the front and from below: the alarectomy has narrowed the base of her nose. And such a long-term result shows what truly matters: that the bridge remains straight and the tip is well-supported even after the entire healing process is complete.

Reason for consultation
Aesthetic. He was bothered by the hump and the size of his nose.
What I observed when evaluating it
A Latin nose whose most striking feature was its width: a very wide base with splayed nostrils. In addition, a hump on the bridge and a long nose with a broad, poorly defined tip.
What was important to preserve
Harmony with the rest of the face: a narrower and thinner nose, but one that was still her own.
My surgical approach
Narrowing the base with an allotomy, which in this nose was the most important thing, removing the hump and refining the tip by giving it support.
Why I chose this technique
The most important feature of this nose was its width, hence the allotomy: it narrows the base. For the rest, I performed a structural ultrasonic rhinoplasty : the ultrasound allows for controlled reduction of the hump, and the structural component supports the tip to prevent it from drooping over time.
What I did during surgery
The hump was reduced using ultrasonic instrumentation, spreader grafts were placed on the dorsum, and a septal extension graft was placed at the tip. An allotomy was performed to narrow the base, with the incision made at the junction of the ala and the face. The septum and turbinates were also addressed in the same surgery. The procedure was performed under TIVA general anesthesia, lasting three hours, with an overnight hospital stay.
What is observed in the result
More than six years later, the base is much narrower, the back is straight, without the hump, and the tip is finer and more supported. This is the result that's important to see before surgery: the one that remains years later.
More examples of wide noses and an explanation of the technique can be found on the Madrid nose surgery page . How I work with Latin and mixed-race noses is explained on the ethnic rhinoplasty page , and why a nose is wide and how the base is narrowed can be found in the wide nose guide.

If you're looking at this case for a specific reason.
Not all noses need ultrasound, nor do they all benefit equally. In our ultrasonic rhinoplasty guide, we explain in which cases the outcome varies.
