This is the most frequent mistake I see in my practice: treating what is actually a drooping eyebrow as excess eyelid skin. If skin is removed from the upper eyelid of a patient with a drooping eyebrow, the eyebrow will drop even further, and the eyes will appear closed instead of open.

When the eyebrow is the culprit, the appropriate solution is to lift it. There are two approaches, and the choice depends on the patient's reason for seeking help: if they are consulting about their eyelids, a direct eyebrow lift is an outpatient procedure with a quick recovery, using an incision just above the hairline; if they are consulting about a complete rejuvenation of the upper third of the face, an endoscopic forehead lift is performed, which is usually done as part of a cervicofacial lift.

Regarding the scar from the direct suspension, it's important to know two things: it really starts to improve from the third month onwards, and it's compatible with subsequent microblading, which is practical for patients with little eyebrow hair.

Advantages of blepharoplasty with the "No Touch" technique

When I talk about the "no-touch" technique, I'm referring to something very specific, and it's worth explaining because the name alone doesn't mean much. It's the principle described by surgeon Glenn Jelks for the lower eyelid: the eyelid muscles are not touched . The incision is made from inside, through a transconjunctival incision, and the orbicularis oculi muscle is left intact.

The reason isn't aesthetic, it's functional. When that muscle is severed, the eyelid loses tone, resulting in a sad look, a rounded eye, and, in the worst cases, ectropion. This is the classic problem with lower blepharoplasty and is very difficult to correct afterward. That's why we prefer not to create it.

Excess skin, with a pinch: if there is also excess skin, it is removed with tweezers just below the eyelashes, drying only the skin. Not a millimeter of muscle.

⇒ Under-eye bags aren't removed, they're repositioned: emptying the eyelid leaves the eye looking sunken and aged. We remove only a portion and use the rest to create a fat flap that we transpose above the tear trough, in a plane below the periosteum. This corrects both the excess bag and the missing tear trough, and it's done using the patient's own tissue.
