A 31-year-old woman with small, widely spaced breasts and a very short lower pole requested a very natural result. The procedure involved augmentation with 270 cc anatomical implants per side, via the inframammary fold, subpectoral incision in dual plane I, and with inframammary fold fixation. The "after" photos are from the 12-month follow-up. With breast augmentation, the results in the first few months are not yet final: the implant is settling and the tissues are adapting to their new position. That's why it's advisable to look at the results after a year. Here, the same views, with the same background and framing, are compared before the operation and at 12 months.

Reason for consultation
I was asking for a very natural result, with a very smooth transition from the top pole.
What I observed when evaluating it
Two things influenced the surgery. A very short lower pole: the distance between the nipple and the inframammary fold was very small and had to be expanded. And very open breasts, with a large distance between them.
What was important to preserve
The naturalness she requested: that the volume be distributed progressively, especially in the upper pole, and that the breasts be in proportion to her body.
My surgical approach
Expand the lower pole with the shape of the implant and bring the two breasts closer to the center with the push-up effect.
Why I chose this technique
I chose anatomical implants because of the smooth transition I requested, and because the shape of these implants expands a short lower pole. Here, the choice is about shape, not just volume. If you're unsure between round or anatomical implants , I explain how to decide in the article.
What I did during surgery
I placed 270cc anatomical implants on each side in a subpectoral position, using dual plane I incision, and secured the inframammary fold with a 3/0 PDS internal bra. Using a push-up effect, I brought the breasts closer together and then applied laser treatment. This is the breast augmentation technique we use with the 24h Recovery Laser protocol.
What is observed in the result
In addition to the volume, the distance between the breasts has been significantly reduced: she started with a very open and widely separated chest, and that's what the push-up bra corrects. In profile, the upper pole rises gradually, without a step, which is what she requested. The push-up bra brings the breasts closer together, but it doesn't change the width of the chest: some of the initial separation remains, as seen in the frontal view.
